All participants in any activity utilizing the Mountainside Youth Center must complete the Youth Center Participation Waiver. One waiver may be used for each family - each individual must be listed on the form.

Participants utilizing the Mountainside Youth Center Fitness Room must complete the Fitness Room Agreement in addition to the Participation Waiver and pay the one-time $20 fee prior to receiving their key-card and use of the Fitness Room
